Approve and/or ratify the Contract Services Action Report 2014-15/02, as submitted.

         
b. Piggyback Authorization - Purchase of School, Office and Janitorial Supplies      (V)  (C)  
 

Authorize the purchase of school, office and janitorial supplies for IUSD using Placentia-Yorba Linda Unified School District Bid#214-04 awarded to Southwest School and Office Supply.

         
c. Piggy Back Authorization – Purchase of Fresh Produce Foods      (V)  (C)  
 

Authorize the purchase of Produce Products for use by Nutrition Services using Colton Joint Unified School District Bid No. 2(13-14) NS awarded to Sunrise Produce Company through June 30, 2015.

All matters of the Community Facilities District Consent Calendar are considered to be routine and will be enacted by the Board in one motion, without prior discussion. At the time the Board adopts the agenda, an item may be removed from the Consent Calendar by the Board, staff, or community for discussion.

         
b. Receive Bids/Award Contracts Portola Springs Elementary School - Increment 2      (V)  (C)  
 

1)  Grant the request for relief from their bid to MS Construction Management, Inc.

2)  Reject the bid of Tectra America, Inc. as being non-responsive.

3) Authorize the Assistant Superintendent of Business Services to award the contracts for the Portola Springs Elementary School Project – Increment 2, for each bid category as listed in the Board report.

         
c. Rate Increase Construction Security Services Various District Locations      (V)  (C)  
 

Authorize the Assistant Superintendent of Business Services to amend the contract with Construction Protective Services to increase the hourly rates for services during the current contract term period, effective July 1, 2014 and expiring on February 7, 2015.

         
d. Ratification of Takeover Agreement Northwood High School Classroom Addition Project      (V)  (C)  
 

Ratify the Takeover Agreement with American Safety Casualty Insurance Company, Inc., as surety for MDE Group, Inc., under Performance Bond No. 1100045141 on the Northwood High School Classroom Addition project. 

         
e. Notices of Completion Jeffrey Trail Middle School - Language Minority Relocation 2014 Project      (V)  (C)  
 

Accept the contracts of the listed contractors for the Jeffrey Trail Middle School – Language Minority Relocation 2014 project as complete and authorize staff to file Notices of Completion with the County of Orange Recorder’s Office.
